with you and your staff from your web pages. One methond of accomplishing
this is by using forms. The patient fills in the blanks and/or selects
items from pull down menus. Once the form is completed the individual
clicks on the submit button. This seamesly sends you an e-mail with information
which was entered on the form.  example
